Billy is a ventriloquist's dummy with a moving jaw to give the impression that he is talking, but he is never used in this way; he can frequently be seen moving around on his own on an old red tricycle.

His face is white with a prominent forehead and chin, his cheeks are painted red in a spiral shape, and his lips are also red, creating a smile on the doll's face. Billy has large black eyes with red irises, and his head is topped with a few straggly black hairs.

Billy is always depicted wearing a black tuxedo, gloves, and a white shirt with a red bow tie and a red handkerchief in his pocket. He can also be seen wearing red Mary Janes for shoes. In the 2003 short, in addition to his usual attire, he also wears a green bowler hat.

The only sound coming from him is an electronic laugh, similar to the sounds made by any typical Halloween toy. In Saw IV, it was revealed that Billy was originally intended to be a smaller and slightly different doll, as John Kramer was going to give it to his son at his birth.
